Our Accredited Practising Dietitians work closely with clients, families, carers and care providers to develop nutrition strategies that are realistic and achievable.

To help us process referrals efficiently, please provide:
Client name and contact details
Date of birth
Residential address
Support at Home provider (if applicable)
Reason for referral
Relevant medical history or diagnosis
Current nutrition concerns
Referrer's contact details
Additional information such as recent weight history, hospital discharge summaries or relevant clinical reports can also assist our dietitians in providing timely support.
Here's what happens when you refer:
All Support At Home clients receive an Initial Assessment and follow-up Reviews for best practice in care.
Step 1.
Referral form received and assessed by a Dietitian.
Step 2.
Quote provided for Initial Assessment and Reviews. Includes Nosh travel zones based on where client lives.
Referrer contacted if additional information required.
Quote accepted by client and/or service provider.
Step 3.
Initial Assessment completed in client’s home or via telehealth where appropriate.
Nutrition care plan developed and shared with client and relevant care team.
Step 4.
Follow up Reviews completed to ensure recommendations implemented and additional nutritional care needs assessed.
FAQs
1. Who should be referred to a dietitian?
Clients may benefit from a dietitian referral if they are experiencing:
Unintentional weight loss
Poor appetite or reduced food intake
Malnutrition or risk of malnutrition
Difficulty managing diabetes
Swallowing difficulties (dysphagia)
Recent hospital admission or illness
Pressure injuries or wounds
Gastrointestinal concerns
Frailty or declining health
Specific dietary requirements related to medical conditions
2. Can clients self-refer?
Yes. We accept referrals from clients, family members, carers, GPs, hospitals, Support at Home providers and other health professionals.
If they do not have Support At Home funding, they can use private health, DVA, NDIS funding or pay full fees.
3. Do you provide home visits?
Yes! We provide in-home consultations across the Perth metropolitan area, allowing clients to receive nutrition support in a familiar environment.
It also allows the dietitians the opportunity to explore with the client where and how they cook, meal preferences and other important information relevant to ageing well at home.
4. What are the Nosh travel zones in the quote?
The Support At Home funding includes travel in the hourly rate. Do not be alarmed by this! Please remember that your service starts from when the referral is made and also includes travel time to and from the client’s home.
The Nosh travel zones are calculated from head office in Stirling. Where possible, the Nosh team will coordinate appointments to be as efficient as possible to reduce the travel time.
